Summary
Strengths: The survey capturespatient reported health status prior to hospitalisation and potentially significant information about integration and coordination of care. There is a comprehensive array of matters on which users can state their opinion and the Likert Scale is used for item responses. Weaknesses: Self-administered questionnaire of self-reported data (e.g. health status and reason for in-patient stay are self-reported) is not methodologically rigorous and does not include outpatient services. This is not a representative sample and there was a high non-response rate.

Data was collected between 2010 and 2011.
The sample size included 21,780 persons who had completed the questionnaire. The completion of the questionnaire was voluntary and in total, 99,000 questionnaires were originally distributed among patients in participating care facilities.
